September 2023 Intake Admission Forms

The admission form below can be filled in at home and you can then either:

• Fill in using Microsoft Word and email the document to school: [email protected]
• Print, fill in and post to school using the details below
• Print, fill in and drop via the school office on Moor Park Avenue

If you are unable to fill out the form using the methods above you can pop in and pick up a pack from the school office.

Note: Please do not fill out the form and send back as photos. Any form filled out by hand needs to be posted or dropped off.

Required Documents

Passport

A copy of your child’s passport is required to ensure we have the correct name, date of birth and nationality on our system. It is also used to cross check with your child’s birth certificate to ensure all the information is correct.

Birth Certificate

A copy of your child’s birth certificate is required in order to crosscheck with the passport and admission form to ensure all the information we have on the system is correct.

Sending your Documents

You may take a photo of your child’s passport and birth certificate or scan a copy and email to [email protected]

Alternatively you may send a photocopy in the post or drop a copy off with the rest of your forms at the address above.

Please do not send originals
